1. Why should I choose Co-Active Physical Therapy and Wellness?

Co-Active believe in one-on one care with quality treatment. You will receive Physical therapy treatment from experienced physical therapist with undivided attention at your home. We believe that hands-on-care and evidence-based techniques have significant value in patient’s recovery.

2. What is a free discovery call?

A free discovery call is something that we offer to people so they can get more information about our services. We want YOU to know as much information as you can without any cost or obligation before you commit to anything. We want you to feel comfortable and confident before you commit to pay us.

3. What will the sessions include?

Your session will start with an initial evaluation consisting of a comprehensive examination. Initial evaluation is 60-90 minutes long. This evaluation will help us to determine the root cause of your concerns. We will develop a comprehensive plan to remediate the root cause. The examination will consist of a series of movement analysis tests specific to your concerns as well as objective measurements. After the initial session, all treatment will be tailored to your individual needs and goals. We will explain to you a detail plan of care, frequency, and duration of treatment and what the end result will look like. Treatment sessions are 45-60 minutes long. Home exercise program will be developed at the end of session.

4. Do I need doctor’s prescription?

As per Texas state law, we can perform an initial evaluation and provide treatment up to 10 business days without a referral. After that doctor’s prescription is required to continue physical therapy treatment. We will send our evaluation to the healthcare provider of your choice and we will take it from there. All you need to do is provide the contact information for that healthcare provider, and we will do the rest. A referral can be provided by an MD, physician’s assistant (PA), nurse practitioner or chiropractor. Referral are valid for one year. If you have a prescription for Physical Therapy, you can bring it in.

5. Why don’t we take your insurance?

We only accept Medicare part B.
Co-Active Physical therapy and wellness is concierge physical therapy services. Concierge means cash-based model. Being in-network with insurance companies means indirectly letting a third party control the quality of care for our patients based on authorizations and approvals.
Co-Active is an out-of-network provider with all insurances. This means higher quality of care dictated by YOU, not your insurance company. We believe in providing highest quality of care to our patients.
Cost is established up-front and agreed upon by you and your physical therapist. No surprises, no bills showing up weeks or months after the completion of therapy. At Co-Active our model demands complete transparency in your healthcare, and that’s the way it should be.

6. How do I pay for the services I receive from Co-Active physical therapy?

You can pay at the time of service and can use cash, check, credit/debit card, or HSA account. Although, we do not personally work with your insurance company, you can submit for reimbursement from your insurance provider after each session with the paperwork we provide you.

7. Are we different from a home health agency?

Yes, we provide outpatient physical therapy services in the comfort of your home, billed under outpatient insurance benefits (Medicare part B) or self-pay if you do not have part B benefits.
Home Health agencies practice under a home care model (billed under Medicare Part A) and can include multiple providers.

8. Why is the cash-based model beneficial to you as a patient?

Cash based model decrease the total number of visits, require less true cost per visit and decrease the overall cost for the entire treatment plan.
Cash based model is not constrained by contracted rates and other insurance contract restrictions. However, you still have options for getting reimbursement for your physical therapy care.
We will have the time and expertise to really dive into understanding your problem in a comprehensive way. We will develop a highly customized game plan for getting you better as quickly as possible.
From our experience, patient ends up spending less money with cash-based model and receive early access, attention, continuity of care, and immediate results.

9. Where are you located?

We come to you! We can meet you at your office, at your home, at your gym, or even at the park. We are mobile to fit your needs and goals.
